How to Activate a New Provider

1. Go to the Application button and choose Provider and the Provider Registration.
2. Add a provider
3. Enter demographic information on the first page and then click on "provider numbers" at the top.
4. Enter the Treating NPI and then click save. You will need to re-visit this screen to fill out the rest of the provider's number for claims.
5.Click Activate Provider and then Activate or subscribe provider.
6. Click Next on the New Provider Subscription page.
7. Select the Provider Type and the Next.
8. Select the proper data set and click next.
9. Select the credit card or fill out the new card information and click Activate.
